Referring your patients

We know you're busy. So you'll be glad to know that due to our reputation for rapid response, combined with our comprehensive, interdisciplinary approach to care, health care providers throughout the region confidently refer their patients to us with one easy phone call.

The Intake Department at the Natick VNA is available for all referrals and information

Weekdays

8:00 AM to 5:00 PM, call 508-653-3081 or 800-834-3997 and press 4 to obtain immediate assistance.

Weekends and Holidays

8:00 AM to 3:00 PM call 508-653-3081 or 800-834-3997 and press 0 to obtain immediate assistance.  

Referral guidelines

We accept referrals from hospitals, physicians, skilled care facilities, assisted/independent living facilities and from patients or family members.

All patients receiving services from our clinical staff must be under the care of a physician licensed in Massachusetts.

You or your staff can call the Natick VNA and give specific orders for care.  A licensed physician must sign all orders for home care services.

The home health service requested must be for skilled need and care, namely; nursing, rehabilitation therapy, nutrition and diet, social services and home care aide for assistance with activities of daily living.

In the event that your patient requires hospitalization, know that you can request the Natick VNA to continue your patient's care upon discharge from your facility.

The Natick VNA's Managed Care Department is staffed by highly qualified registered nurses who are familiar with the process and requirements of HMOs and other managed care insurance companies. They are in constant communication with the insurance authorizers regarding the requested services, and they try to obtain the best care possible for each patient.
